Job summary

Evolve Therapy Services in Lowell, MA is seeking a dedicated Physical Therapist to join our in-house skilled nursing facility team. You will plan, direct, and deliver physical therapy services, ensuring safety and compliance with CMS guidelines and the organization's standards.

Responsibilities include evaluating residents, developing short and long-term treatment plans, collaborating with an interdisciplinary team, and ensuring accurate documentation in NetHealth EMR.

Qualifications

• Minimum of Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents per standards and procedures.
• Adhere to documentation and billing procedures for therapy services.
• Evaluate residents and plan treatment with goals and methods.
• Conduct routine resident screenings to determine need for intervention.
• Develop treatment plans with short and long-term goals.
• Complete CMS-compliant documentation in NetHealth EMR.
