Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapist to join our in-house Skilled Nursing Facility partner in Madison, NJ. This full/part-time or PRN role focuses on planning, organizing, and directing Physical Therapy services to ensure high-quality resident care.

Responsibilities include evaluating residents, developing treatment plans, documenting for CMS standards, and collaborating with families and the interdisciplinary team to maximize functional outcomes.

Qualifications

• Minimum of Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.
• Assume responsibility for ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents according with established standards of practice and department procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Demonstrate sound judgement in evaluation, planning, implementation and follow-up of resident therapy programs.
• Conduct screenings of residents to determine need for intervention/treatment.
• Develop treatment plans by setting short and long-term goals and methods to achieve them.
• Interpret and communicate evaluation findings and recommendations to residents, families, and interdisciplinary team members.
• Complete CMS-compliant documentation in NetHealth EMR systems.
