Job summary

Evolve Therapy Services, LLC seeks a Physical Therapist for an in-house Skilled Nursing Facility in Pahokee, FL. This role offers Full Time, Part Time, or PRN schedules and focuses on delivering high-quality therapy to residents in a compliant setting.

Responsibilities include evaluating residents, planning and implementing treatment programs, documenting care, and communicating progress to families and the interdisciplinary team, using NetHealth EMR.

Qualifications

• BS in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Commitment to ongoing professional development.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and department procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Develop treatment plans with short and long-term goals and methods to achieve identified goals.
• Communicate evaluation findings and treatment recommendations to residents, families, and interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR.

Education

Bachelor of Science Degree in Physical Therapy from an accredited program
