Job summary

Evolve Therapy Services is seeking a Physical Therapy Assistant to join an in-house Skilled Nursing Facility in Pahokee, FL. This PRN position supports the licensed Physical Therapist in delivering resident-centered therapy services.

Qualifications

• Associate's degree in physical therapy is required.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services under the direct supervision of the Physical Therapist.
• Conduct resident screenings at regular intervals to determine need for intervention.
• Assist in evaluation and treatment planning with the Physical Therapist.
• Prepare and modify patient schedules with the team and adhere to CMS documentation.
