Job summary

Chase Center Rehabilitation and Nursing Services in Logansport, IN is seeking a dedicated Physical Therapist (PT) to join our in-house rehab team on a PRN or full-time basis.

You will perform comprehensive PT evaluations, develop individualized plans, deliver evidence-based interventions, monitor progress, document outcomes, and collaborate with the interdisciplinary team to improve mobility, independence, and quality of life for residents in a skilled nursing setting.

Qualifications

• Current Physical Therapist (PT) license in Indiana.
• Graduate of an accredited Physical Therapy program.
• Must be at least 18 years of age.
• Skilled nursing or long-term care experience preferred.

Responsibilities

• Perform comprehensive physical therapy evaluations.
• Develop and implement individualized treatment plans to improve mobility, strength, balance, and functional independence.
• Deliver evidence-based therapeutic interventions.
• Monitor resident progress, update goals, and adjust treatment plans as needed.
• Accurately document evaluations, treatments, and outcomes in compliance with all regulatory requirements.
• Collaborate with the interdisciplinary care team to optimize resident outcomes.
• Educate residents, families, and caregivers on therapy goals, progress, and home exercise programs.
