Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapist for an in-house Skilled Nursing Facility in Omaha, NE. This PRN role requires evaluating residents, developing treatment plans, and collaborating with an interdisciplinary team to ensure high-quality resident care.

The successful candidate will maintain CMS-compliant documentation in NetHealth EMR and participate in ongoing professional development to sustain licensure.

Qualifications

• Must hold a Bachelor of Science Degree in Physical Therapy.
• Must have an active Physical Therapy license in state(s) of practice.
• Engage in ongoing continuing education and professional development.

Responsibilities

• Provide physical therapy services to residents per standards and procedures.
• Document and bill physical therapy services per CMS expectations.
• Evaluate residents and develop treatment plans with short- and long-term goals.
• Conduct screenings to identify intervention needs and plan accordingly.
• Communicate evaluation findings and treatment plans to residents and interdisciplinary team.
