Job summary

Evolve Therapy Services is seeking a Physical Therapist for an in-house Skilled Nursing Facility in Rochester, MN. This full-time or PRN role focuses on planning, directing, and delivering physical therapy services in compliance with standards and CMS expectations.

The therapist will evaluate residents, develop treatment plans, document care, and collaborate with an interdisciplinary team to ensure high-quality resident outcomes.

Qualifications

• BS in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards and procedures.
• Document and bill physical therapy services per company policies.
• Evaluate residents to determine needs and develop treatment plans.
• Develop short- and long-term goals and methods to achieve them.
• Communicate evaluation findings and plans to residents, families, and interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR.
