Qualifications

• Degree and licensure required to practice physical therapy.
• Ongoing continuing education to maintain licensure.
• Active license in state(s) of practice required.

Responsibilities

• Provide PT services to residents per standards and procedures.
• Document and bill services in CMS-compliant systems.
• Evaluate residents and develop treatment plans with goals.
• Conduct resident screenings and determine need for intervention.
• Communicate findings to residents, families, and the care team.
• Complete compliant, medically necessary documentation.
