Job summary

Evolve Therapy Services is hiring a Physical Therapist for an in-house Skilled Nursing Facility in New Haven, CT. This PRN role focuses on delivering high-quality therapy services and coordinating with the care team to improve resident outcomes.

Responsibilities include assessments, plan development, documentation, and collaboration with families and the interdisciplinary team. Active PT license and ongoing CE are required.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Commitment to ongoing professional development.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards and procedures.
• Adhere to documentation and billing procedures for therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation, and follow-up of resident therapy programs.
• Conduct screening of residents to determine need for intervention/treatment.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop treatment plans with short and long-term goals.
• Communicate evaluation findings and treatment plans to residents, families, and team members.
• Complete compliant and medically necessary CMS documentation in NetHealth EMR systems.
