Job summary

The Arboretum of Winnie in Richardson, TX is seeking a Full-Time Physical Therapist (PT) to join our in-house team of professionals. You will provide person-centered care to help residents improve mobility, strength, and quality of life.

As a PT, you will assess patients, develop and implement treatment plans, document thoroughly, and collaborate with the interdisciplinary team to ensure high-quality care. Texas license and SNF experience preferred.

Qualifications

• Current Texas PT license in good standing.
• Experience in SNF or long-term care preferred.
• Knowledge of physical therapy treatment practices.
• Experience with Net Health EHR preferred.

Responsibilities

• Consult with patients to assess their physical condition and functional limitations.
• Evaluate and interpret patient assessments and test results.
• Develop individualized treatment plans using a variety of therapy techniques.
• Create personalized rehabilitation programs tailored to each patient’s goals.
• Administer medically prescribed physical therapy treatments to relieve pain, restore function, and promote healing.
• Ensure all treatments are delivered in accordance with the established plan of care.
• Monitor patient response to treatment interventions and modify approaches as clinically appropriate in collaboration with the interdisciplinary team.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with interdisciplinary team members to ensure high‑quality patient care.
• Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations.
• Comply with all applicable rules, regulations, and procedures.
• Attend required meetings as designated by the Director of Rehab
