Job summary

CareOne is seeking a Physical Therapist to join our in-house rehab team at CareOne Northampton, MA. In this role you will provide resident care at the highest level and collaborate daily with nursing and clinical staff to improve patient outcomes.

Qualifications

• Master's or Doctorate Degree in Physical Therapy.
• Current and Active Physical Therapist license within the state of operations.
• Prefer experience/knowledge in older adult rehabilitation.
• Prefer knowledge in governmental and regulatory guidelines in designated healthcare settings.

Responsibilities

• Provides and directs resident rehabilitation care within general accepted practice methods and meets standards.
• Evaluates residents and establishes effective treatment plans.
• Coordinates supervision of treatment plans with all staff and other disciplines.
• Screens residents according to schedules and practices.
• Provides feedback to the supervisor on clinical and rehabilitation issues.
• Participates in center directed processes and services.
• Performs clinical documentation to meet requirements.
• Maintains billing records to meet requirements.
• Participates in in-service training programs as requested.
• May travel to other facilities to provide services on a limited basis.

Skills

Experience in older adult rehab

Knowledge of regulatory guidelines

Education

Master's degree in Physical Therapy

Doctorate degree in Physical Therapy

Physical Therapist license (active)
