Job summary

Evolve Therapy Services, LLC in Wilmington, NC seeks an Occupational Therapist to provide in-house services at a Skilled Nursing Facility. This role offers Full Time, Part Time, or PRN options and focuses on delivering high-quality resident care in line with current standards.

Responsibilities include screening, evaluation, treatment planning, documentation, and coordination with the care team. OT degree and active licensure are required; CEUs to maintain licensure encouraged.

Qualifications

• Degree in Occupational Therapy.
• Must have an Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education and professional development to maintain current licensure and certification

Responsibilities

• Conduct regular screening of residents to determine the best intervention.
• Evaluate residents to inform treatment plans and set goals.
• Communicate evaluation findings and plans to residents and care team.
• Establish individual therapy schedules with the team.
• Document and bill for occupational therapy services.
• Oversee COTA as needed and ensure documentation and treatments meet reimbursement standards.
