Job summary

Evolve Therapy Services is hiring an Occupational Therapist for an in-house Skilled Nursing Facility in Durham, NC. The role is full-time and focuses on planning, organizing, and directing occupational therapy services within federal, state, and local standards.

You will screen residents, develop treatment plans, communicate outcomes to the care team, and oversee COTA while ensuring proper documentation and compliant reimbursement practices.

Qualifications

• Degree in Occupational Therapy.
• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine best intervention for each patient.
• Evaluate residents to inform and develop treatment plans; set short- and long-term goals.
• Communicate evaluation findings, treatment plans, and recommendations to residents, families, and team members.
• Establish individual resident therapy schedules with the team.
• Conduct therapy per treatment plan and ensure proper documentation and billing.
• Perform other duties to effectively treat patients.
• Oversee COTA and ensure documentation and treatments meet reimbursement standards.
