Job summary

Evolve Therapy Services, LLC is hiring an Occupational Therapist for an in-house Skilled Nursing Facility in North Carolina. The role supports a range of full time, part time, or PRN positions within a facility setting.

You will plan, direct, and coordinate OT services, ensuring quality resident care in compliance with federal, state, and local standards. Active licensure and ongoing CE are required for continued practice.

Qualifications

• Degree in Occupational Therapy.
• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education to maintain licensure.

Responsibilities

• Conduct regular screening of residents to determine best intervention or treatment.
• Evaluate residents to develop treatment plans with goals.
• Interpret and communicate evaluation findings to residents, families, and team.
• Establish resident therapy schedules with team members.
• Document and bill therapy services according to plan.
• Oversee COTA and ensure procedures meet clinical standards.
