Job summary

Evolve Therapy Services seeks an Occupational Therapist to support an in-house Skilled Nursing Facility in Pahokee, FL. The role involves planning and directing occupational therapy services to residents in compliance with applicable standards, under facility direction.

The ideal candidate will hold an active OT license, be able to organize schedules with the team, and commit to ongoing professional development while delivering high-quality care to residents.

Qualifications

• Must hold an active Occupational Therapy license in practicing state(s).
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Screen residents to determine best interventions.
• Evaluate residents and develop treatment plans with goals.
• Communicate findings and plans to residents, families, and team.
• Schedule resident therapy with other team members.
• Provide therapy per plan and ensure documentation and billing.
