Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Memphis, TN. This full-time role focuses on planning, directing, and delivering occupational therapy services in accordance with federal, state, and local standards to ensure high-quality resident care.

Responsibilities include screening residents, executing treatment plans, coordinating with the care team, documenting services, and ensuring compliant billing.

Qualifications

• Must have active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Conduct regular screenings to determine best intervention for residents.
• Implement treatment plans and follow short- and long-term goals.
• Interpret and communicate evaluation findings and plans to residents, families, and the interdisciplinary team.
• Establish resident therapy schedules with team members.
• Ensure therapy services are documented and billed per plan.
• Perform duties required to effectively treat patients.
• Adhere to CMS documentation standards daily with point-of-care documentation.
