Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant to join our in-house Skilled Nursing Facility in Memphis, TN. This full-time role plans, organizes, and directs OT services in accordance with standards to ensure high-quality resident care.

Responsibilities include screening residents, developing treatment plans with the supervising therapist, communicating findings to families and the care team, scheduling therapy sessions, and maintaining documentation and billing

Qualifications

• OTA degree required from accredited program.
• Active licensure to practice OTA.
• Commitment to ongoing professional development and licensure maintenance.

Responsibilities

• Screen residents to determine best intervention and treatment plan.
• Implement treatment plans with supervision and monitor progress toward goals.
• Communicate findings and recommendations to residents, families, and care team.
• Coordinate therapy schedules with interdisciplinary team members.
• Document therapy services and assist with CMS-compliant billing.
• Perform other duties as assigned to support resident care.
• Ensure documentation adheres to CMS standards on a daily basis.

Education

Associate's degree in Occupational Therapy

Active OTA licensure in state(s) of practice

Ongoing continuing education and professional development

Certified Occupational Therapy Assistant for an In-house Skilled Nursing Facility

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice
• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off of the established plan of care by supervising therapist
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications

• Associate's degree in Occupational Therapy
• MUST have active OTA licensure in state(s) of practice
• Ongoing continuing education and professional development to maintain current licensure and certification
