Job summary

Evolve Therapy Services is seeking a compassionate Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Shakopee, MN. The position is PRN and involves delivering occupational therapy services on-site.

You will plan, organize, develop, and direct OT services, screen residents, implement treatment plans, document and bill, and collaborate with the interdisciplinary team to ensure quality resident care.

Qualifications

• Must hold OTA licensure in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine best intervention.
• Perform treatment plans following goals set by supervising therapist.
• Interpret and communicate evaluation findings to residents, families/caregivers, and team members.
• Establish individual resident therapy schedules with team members.
• Conduct therapy according to plan and ensure proper documentation and billing of OT services.
• Comply with CMS documentation standards daily.
