Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant (COTA) for an in-house Skilled Nursing Facility in Princeton, NJ. Part-time or PRN positions available.

The COTA will plan and deliver OT services under supervision, screen residents, develop treatment plans, document care per CMS standards, and collaborate with the interdisciplinary team. Active OTA licensure in the state(s) of practice is required.

Qualifications

• Must have OTA licensure in state(s) of practice.
• Associate's degree in Occupational Therapy.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Screen residents to identify best interventions.
• Execute treatment plans under supervision.
• Communicate findings to residents, families, and team.
• Schedule therapy sessions with the care team.
• Document and bill therapy services per CMS standards.
• Perform duties as assigned to support patient care.
• Ensure CMS documentation standards daily.
