Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant to support an in-house Skilled Nursing Facility in Altoona, PA. This position is available as Full Time or PRN and focuses on delivering quality OT services in a facility setting.

You will plan, implement, and document OT interventions while collaborating with an inter-disciplinary team and ensuring licensure standards and CMS guidelines are followed.

Qualifications

• Requires an Associate's degree in Occupational Therapy.
• Active OTA licensure in practicing states.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine the best intervention or treatment.
• Perform treatment plans and follow goals per plan of care.
• Communicate evaluation findings to residents, families, and team.
• Establish resident therapy schedules with the team.
• Document and bill OT services and ensure CMS compliance.
