Evolve Therapy Services, LLC seeks a Certified Occupational Therapy Assistant to join an in-house Skilled Nursing Facility in Altoona, PA. This PRN role supports resident care under supervision, helping to maintain high-quality therapy services.

Responsibilities include screening residents, implementing treatment plans, documenting care, and collaborating with an interdisciplinary team. Requires an Associate's degree and active OTA licensure, with ongoing CE to maintain credentials.

Formación

• Must have active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsabilidades

• Screen residents to identify the best interventions per scope of practice.
• Perform treatment plans and follow short- and long-term goals with supervision.
• Interpret and communicate evaluation findings, plans, and recommendations to residents, families, and the team.
• Establish resident therapy schedules with input from team members.
• Deliver therapy per plan and ensure proper documentation and billing.
• Perform all other duties to effectively treat patients.
• Comply with CMS documentation standards daily with point-of-care notes.
