Evolve Therapy Services in Muskego, WI is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility. This full-time role supports planning, directing, and delivering occupational therapy services to residents in accordance with federal, state, and local standards.

You will conduct screenings, implement treatment plans, communicate findings to residents and families, coordinate schedules, ensure proper documentation and billing, and comply with CMS standards.

Qualificações

• Must have OTA licensure in state(s) of practice.
• Associate's degree in Occupational Therapy is required.
• Ongoing continuing education to maintain licensure.

Responsabilidades

• Plan, organize, develop, and direct Occupational Therapy Services in facility.
• Screen residents to identify intervention based on scope of practice.
• Implement treatment plans and document/follow CMS standards daily.
• Communicate evaluation findings and recommendations to residents and care team.
• Establish resident therapy schedules in coordination with team.
• Ensure proper billing and documentation for services rendered.

Formação académica

Associate's degree in Occupational Therapy

Active OTA licensure in state(s) of practice
