Job summary

Evolve-Therapy-Services seeks an Occupational Therapist to lead in-house therapy services at a Skilled Nursing Facility in the Chicago area. This role offers Full Time with a flexible schedule, plus relocation assistance, as well as Part Time or PRN options.

You will screen residents, develop treatment plans, supervise COTAs, and coordinate with the care team to ensure high-quality resident care and accurate documentation for billing.

Qualifications

• Must hold an active OT license in the practicing state.
• Degree in Occupational Therapy.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Screen residents to determine best interventions for each patient.
• Evaluate residents to inform and develop treatment plans with goals.
• Communicate findings and plans to residents, families, and the care team.
• Establish individual therapy schedules with the team.
• Deliver therapy per plan and ensure documentation and billing.
• Oversee COTA and ensure documentation and treatments meet standards.
