Job summary

Advanced Health Care of Colorado Springs is seeking a motivated Physical Therapist to join our in-house team as a PRN provider, serving residents in a geriatric short-term rehab setting with 34 beds. The role includes PT evaluations and treatment, as well as supervision of in-house PTA staff. Must hold a Physical Therapist license.

Qualifications

• Current state licensure or eligibility for licensure as a Physical Therapist (PT) or a Physical Therapist Assistant (PTA).
• Strong clinical reasoning and decision-making skills for evaluating and treating diverse conditions.
• Ability to build rapport with residents with empathy and compassion.

Responsibilities

• Evaluate residents' physical abilities and rehabilitation needs through assessments.
• Develop personalized treatment plans based on assessments and goals.
• Implement interventions to maximize functional independence and mobility.
• Provide one-on-one PT sessions using a range of techniques.
• Monitor progress and adjust plans to optimize outcomes.
• Collaborate with physicians, nurses, and other therapists to coordinate care.
• Educate residents and families on exercises and safety techniques.
• Document evaluations, progress notes, and discharge summaries per policy.
• Participate in care conferences and team meetings to support care planning.
• Demonstrate professionalism and teamwork.
