Advanced Health Care of Glendale is seeking a Physical Therapist to deliver rehabilitative services to residents in a geriatric setting. The role encompasses evaluating patients, planning individualized treatment, and delivering one-on-one PT sessions to restore mobility and function.

You will collaborate with the care team, document progress, and educate residents and families on exercises and safety at home. A supportive, professional work environment and flexible scheduling are offered.

Competenze

• Must hold PT license or be eligible for licensure.
• Experience in a skilled nursing facility or healthcare setting preferred.
• Strong clinical reasoning and decision-making skills for diverse conditions.
• Ability to build rapport with residents through a compassionate demeanor.
• Excellent interpersonal and communication skills with residents, families, and team.

• Evaluate residents' physical abilities, functional status, and rehabilitation needs through assessments.
• Develop personalized treatment plans based on assessments and goals.
• Implement therapies to improve mobility and functional independence.
• Provide one-on-one physical therapy sessions.
• Monitor progress and adjust treatment plans as needed.
• Collaborate with physicians, nurses, and other therapists to coordinate care.
• Educate residents and families on exercises and home programs.
• Document evaluations, progress notes, and discharge summaries per policy.
• Participate in care conferences and team meetings.
