Paycom - ATS is seeking a Violence Intervention Liaison to provide case management support to individuals in the Hospital Violence Intervention Program. You will conduct in-hospital and home visits, mentor at-risk youth, coordinate with partners, and advocate for access to social services and job opportunities.

The role emphasizes trauma-informed care, HIPAA compliance, and collaboration with hospital staff, law enforcement, schools, and community organizations to reduce violence and support

Competenze

• Requires ability to work with at-risk youth and families from diverse backgrounds.
• Must maintain HIPAA and hospital confidentiality guidelines.
• Experience working with healthcare settings is strongly preferred.

Mansioni

• Provide interventions, counseling, and advocacy to hospitalized youths/young adults affected by violence.
• Develop client service plans and conduct initial screens and visits during hospitalization and after discharge.
• Coordinate access to social services, education, and job opportunities; build positive relationships with enrollees.
• Engage with hospitals, schools, law enforcement, social services, courts, and community organizations to prevent retaliation and repeat violence.
