Qualifications

• Basic computer skills required.
• Ability to keep accurate and timely records.

Responsibilities

• Conducts daily hospital rounds connecting with participants still hospitalized.
• Provides weekday evening and overnight coverage.
• Responds to notifications of eligible participant arrival; arrives within 30 minutes; stays as needed.
• Conducts initial bedside visit and crisis response for patient and family support.
• Provides comfort and education about emotional consequences following violent injury.
• Builds rapport, assesses risk and needs, and determines need for community-based follow-up services.
• Guides participant through goal plan and connects to community resources.
• Maintains ongoing contact with youth and family until discharge to Next Step Case Manager.
