Job summary

TeamUP Counseling is seeking an intensive in-community therapist to provide in-home clinical services to young individuals with behavioral health challenges. The role requires a master's degree and an active New Jersey LSW/LCSW or LAC/LPC license. Candidates should have at least 1 year of relevant experience and a reliable transportation.

Qualifications

• Must have an active New Jersey LSW/LCSW or LAC/LPC license.
• Minimum 1 year of experience working with young people with behavioral health needs.
• Access to reliable transportation to commute to and from clients' homes.

Responsibilities

• Conduct weekly individual and/or family sessions to young people.
• Develop and implement treatment plans.
• Collect and document medical records in compliance with HIPAA.
• Collaborate with case management teams to coordinate care plans.
• Engage with families and caregivers through home & community visits.
• Maintain ongoing professional development.
