Job summary

Personic Health in Illinois is seeking a dedicated Medical Assistant to support Nurse Practitioners and clinical teams delivering in-home wound care. You will help with visits, documentation, wound care logistics, and telehealth support, while maintaining HIPAA-compliant records.

Responsibilities

• Assist providers during in-home patient visits, including preparing supplies, and documenting care.
• Facilitate telehealth visits in the home as needed.
• Support the clinical team with wound photography, staging, dressing setup, and record keeping.
• Maintain accurate, timely electronic health records (EHR) documentation.
• Ensure all necessary supplies and equipment are stocked, organized, and available for patient visits.
• Serve as a professional and compassionate point of contact for patients, families, and facility partners.
• Coordinate with scheduling, operations, and clinical leadership to support efficient daily workflows.
• Follow all Policy and HIPAA regulations and infection control standards.
• Assist with follow-ups, care coordination, and general administrative tasks as needed.
