Personic Health is seeking a dedicated Medical Assistant to support Nurse Practitioners and clinical teams in in-home wound care services in Westchester County, NY. The role focuses on preparing supplies, documenting care, and facilitating telehealth visits, while maintaining precise electronic health records.

• Previous wound care or home health experience valued.
• Strong medical terminology and documentation skills.

Mansioni

• Assist providers during in-home patient visits, including preparing supplies and documenting care.
• Facilitate telehealth visits in the home as needed.
• Support the clinical team with wound photography, staging, dressing setup, and record keeping.
• Maintain accurate, timely electronic health records (EHR) documentation.
• Ensure all supplies and equipment are stocked and available for visits.
• Serve as professional point of contact for patients, families, and facility partners.
• Coordinate with scheduling, operations, and clinical leadership to support workflows.
• Follow HIPAA regulations and infection control standards.
• Assist with follow-ups, care coordination, and general administrative tasks.
