Job summary

Phoenix Home Care is seeking a motivated RN or LPN to join our In-Home team in Springfield, MO. This part-time role focuses on delivering high-quality nursing care in clients' homes and enhancing their daily living with compassionate support.

Responsibilities include patient assessments, vitals, medication management, and caregiver supervision as needed. We offer flexible scheduling and a supportive in-home team culture.

Qualifications

• Active MO RN or LPN license
• Home care experience preferred
• Strong organizational and communication skills
• Valid driver’s license
• Compassionate customer service

Responsibilities

• Complete admission assessments and reassessments for In-Home caregiver population.
• Provide nurse visits for adult and senior population: Taking vitals, medication management, monthly diabetic nails, and labs.
• May provide supervisory visits for caregiver services
• Provide instruction/ education on medication
• Good time management, detail oriented and self-starter
