Signify Health, part of CVS Health, is hiring a Full Time Clinician (NP or PA) to perform in-home health evaluations across a defined coverage area. You will visit members at their homes, review medications and medical histories, perform physical assessments, and coordinate with health plans for appropriate follow-up care.

This role requires active licenses, board certification, and multi-state licensure as needed, with travel up to 100% and overnight stays as required.

Qualificações

• Active, unrestricted licenses in the relevant coverage areas.
• Board certification is required.
• Ability to obtain and maintain multi-state licenses as needed.
• Willingness to travel to multiple counties within the state.

Responsabilidades

• Conduct in-home health evaluations with patient health history review.
• Travel locally across defined coverage areas; overnight stays as needed.
• Use iPad-based clinical workflow; collect and record diagnostic data.
• Coordinate with health plans to connect members with appropriate care.
• Provide diagnostic and preventive services per program guidelines.
