Signify Health, part of CVS Health®, is seeking a Full Time Clinician (NP/PA) to perform In-Home Health Evaluations across a defined coverage area. The role involves travel, weekends as needed, and lodging as required.

You will engage members at home, review medication history, and conduct physical evaluations using a patient-centric approach. Requirements include active, unrestricted NP/PA licenses, board certification, and the ability to obtain multi-state licenses.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification is required.
• Able to obtain and maintain multi-state licenses as needed.
• Willing to travel locally and across states as required.

Responsabilités

• Bring your clinical skills to in-home visits across a defined coverage area.
• Conduct comprehensive health evaluations including review of meds and history.
• Perform diagnostic tests as ordered by the health plan when applicable.
• Use iPad and clinical workflow; no prescribing or altering current regimens.
• Be flexible to travel up to 100% and conduct virtual visits as needed.
