Job summary

Signify Health, part of CVS Health, seeks a Full Time Clinician (NP or PA) to provide in-home health evaluations across a defined coverage area, traveling up to 100% of the time. The role centers on building trust with members and delivering personalized care at home.

Visits last about 45 minutes and include medication/history review and optional diagnostic tests. Licensure and board certification are required, with multi-state capability as needed.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Able to obtain and maintain multi-state licenses in coverage area, as required.

Responsibilities

• In-home health evaluations for health plan members, using an iPad and clinical workflow.
• Travel locally and within licensed states up to 100% of the time.
• Provide diagnostic and preventive services as indicated by the health plan and documentation.
• Conduct virtual visits as needed and bridge gaps in care.
