Job summary

Signify Health, part of CVS Health, seeks a Full Time Clinician (NP or PA) to provide in-home health evaluations across a defined coverage area. You’ll travel up to 100% of the time, visiting members in their homes and coordinating care.

Using an iPad and clinical workflow, you perform med history review, physical evaluations, and discuss findings with health plans. Overnight travel and multi-state licensure may be required.

Qualifications

• Active, unrestricted licenses in coverage areas.
• Board certification required for NP or PA.
• Ability to obtain and maintain multi-state licenses as needed.
• Ability to cover all counties within state and flex to other areas.

Responsibilities

• Provide in-home health evaluations to members in their homes.
• Travel up to 100% of the time across defined coverage area.
• Use an iPad and clinical workflow to document visits and findings.
• Coordinate with health plans and bridge gaps in care.
