Job summary

Aveanna Healthcare is seeking a Direct Support Professional to provide one-on-one support in home and community settings for individuals with intellectual and developmental disabilities. You will help promote emotional, physical, and personal well-being while supporting daily living activities.

You will implement ISP strategies, complete documentation (EVV, daily service notes), and work with a multidisciplinary team to maintain the Individual Support Plan.

Qualifications

• High school diploma or equivalency is required.
• Active driver’s license and current auto insurance/registration/inspection if transporting.
• 18 years or older and flexible hours.
• Ability to work with individuals with intellectual/developmental disabilities.

Responsibilities

• Provide one-on-one support to individuals with IDD to promote wellbeing.
• Implement ISP strategies to help individuals achieve outcomes.
• Complete EVV and daily service notes documenting services.
• Assist with ISP development and maintenance with the interdisciplinary team.
• Report changes in condition to the manager promptly.
• Maintain confidentiality and professional boundaries.
• Help individuals reach goals and develop life skills.
• Support community participation and daily living activities.
