Job description

Position Overview

Working in the home and/or a community environment, the Direct Support Professional is responsible for delivering quality one‑on‑one services to individuals with intellectual and/or developmental disabilities, working towards their outcome statements as outlined in the Individual Support Plan. Care may also involve daily living activities and communication with members of the individual’s multidisciplinary team.

Essential Job Functions

• Professionally support adult or pediatric individuals with intellectual and/or developmental disabilities in their homes, workplaces, or other community settings, promoting their emotional, physical, and personal well‑being.
• Implement strategies to help the individual achieve outcomes outlined in the Individual Support Plan (ISP).
• Complete necessary documentation to substantiate services delivered (e.g., electronic visit verification and daily service notes).
• Assist the individual and interdisciplinary team in developing and maintaining the ISP.
• Immediately report any changes in the individual's condition to the manager.
• Ensure all documentation complies with state and program regulations, Aveanna HealthCare policies, and applicable accreditation standards.
• Adhere to confidentiality standards and professional boundaries at all times.
• Guide individuals toward greater independence through instruction, role modeling, and support.
• Seek opportunities for the individual to experience life in their homes and communities based on hobbies and interests.
• Support the individual’s preferences and honor their interests.
• Assist in developing life‑skill competencies (e.g., social interaction, cooking, housekeeping, budgeting, personal hygiene).
• Help individuals live an everyday life and ensure their health and safety.

Requirements

• Desire to help individuals live life to its fullest.
• High school diploma or equivalent.
• Active driver’s license, current automobile insurance, car registration, and insurance inspection if transporting individuals.
• Willingness to accompany those served into the community and provide support during interactions while respecting civil and human rights.
• Must be 18 or older.
• Ability to work flexible hours as needed.
• Eligibility for State Police Criminal Record Check and Child Abuse History Clearance.
• FBI clearance required for individuals who do not meet PA residency requirements.
