Job summary

Trinity Health At Home – Muskegon is seeking a part-time Home Health Speech Therapist to provide in-home speech and audiology diagnostic and therapeutic services under physician orders, compliant with the state licensure requirements.

You will assess clients, develop treatment plans with physicians, document thoroughly in the EMR, and coordinate care with the care team. The role offers guaranteed hours, competitive pay, and low-cost benefits with growth opportunities.

Qualifications

• Meets the educational and experience requirements for a CCC-SLP by ASHA or is in the process of accumulating supervised experience for certification.
• Registration or licensure to practice as a speech pathologist in state of practice, if applicable.
• Maintains clinical practice skills and adheres to agency policies.
• Functions with minimal supervision and maintains professional relationships with clients and families.
• Possesses a valid driver’s license and reliable transportation.
• Documents observations, evaluations, and interventions in the point-of-care system per policy.
• Timely submission of client data to the office for records.

Responsibilities

• Prioritizes visits and plans visits based on client needs, orders, and resources.
• Maintains knowledge of applicable laws, ethics, and agency policy; demonstrates ethical behavior.
• Assesses clients at admission and required time points to determine speech/audiology needs.
• Develops assessment-based plan of treatment with physician and client; revises plan as needed.
• Makes referrals to other disciplines and coordinates care for effective service delivery.
