Job summary

Trinity Health At Home – Muskegon is seeking a Home Health Speech Therapist to provide in-home diagnostic and therapeutic speech services as authorized by the physician and in compliance with state practice acts. You will assess and treat speech and language needs, document progress, and coordinate with the care team to ensure seamless patient care.

The role emphasizes consistent workloads, supportive leadership, and opportunities for growth within a faith-based, not-for-profit home care

Qualifications

• Meets the educational and experience requirements for a Certificate of Clinical Competence in Speech Pathology (ASHA) or is in the process of accumulating supervised experience for certification.
• Registration or license to practice as a speech pathologist in the state of practice if applicable.
• Maintains clinical practice skills and adheres to agency policies and procedures.

Responsibilities

• Provide in-home diagnostic and therapeutic speech-language pathology services to clients as authorized by physician and state act.
• Document client progress and maintain records in the EMR system.
• Coordinate with care teams and schedule visits to align with the plan of care and PTO planning.

Minimum Qualifications:

• Meets the educational and experience requirements for a Certificate of Clinical Competence in Speech Pathology granted by the American Speech and Hearing Association; or, meets the educational requirements for certification and is in the process of accumulating the supervised experience required for certificate.
• Registration or license to practice as a speech pathologist in the state of practice if applicable.
• Must accept responsibility for maintaining clinical practice skills, learning and adhering to agency’s policies and procedures on an on-going basis.
• Must be able to function in a practice environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with the agency’s clients and their families.
• Must have current Driver’s license and reliable transportation to and from work site.
• Takes appropriate steps to assure timely submission of client data to the office for inclusion in the client record (by timely electronic transmission of data, or when appropriate by fax or mail).
• Works with scheduling to assure visits and client care coverage are scheduled in accordance with the plan of care and plans ahead for PTO or other time off.
