Job summary

Trinity Health At Home – Muskegon seeks a part-time Home Health Speech Therapist to provide in-home diagnostic and therapeutic services for clients as ordered by physicians and compliant with state practice acts.

You’ll work with the care team to ensure consistent, high‑quality care, maintain documentation in our EMR system, and support family education and home health goals in a compassionate, patient‑centered approach.

Qualifications

• CCC-SLP or in process of supervised experience for certification.
• License to practice speech pathology in the state of practice.
• Maintain clinical practice skills and adhere to agency policies.
• Ability to work with minimal supervision.
• Valid driver’s license and reliable transportation.
• Timely submission of client data.
• Coordinate visits with plan of care and PTO planning.

Responsibilities

• Provide in-home speech pathology diagnostic and therapeutic services as authorized by physician.
• Collaborate with physicians and care team to ensure quality, compliant care.
• Document services accurately in EMR and coordinate with scheduling.

Minimum Qualifications:

• Meets the educational and experience requirements for a Certificate of Clinical Competence in Speech Pathology granted by the American Speech and Hearing Association; or, meets the educational requirements for certification and is in the process of accumulating the supervised experience required for certificate.
• Registration or license to practice as a speech pathologist in the state of practice if applicable.
• Must accept responsibility for maintaining clinical practice skills, learning and adhering to agency’s policies and procedures on an on-going basis.
• Must be able to function in a practice environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with the agency’s clients and their families.
• Must have current Driver’s license and reliable transportation to and from work site.
• Takes appropriate steps to assure timely submission of client data to the office for inclusion in the client record (by timely electronic transmission of data, or when appropriate by fax or mail).
• Works with scheduling to assure visits and client care coverage are scheduled in accordance with the plan of care and plans ahead for PTO or other time off.
