Job summary

Trinity Health At Home – Muskegon seeks a part-time Home Health Speech Therapist to deliver in-home diagnostic and therapeutic speech services as authorized by physicians and in line with state licensure requirements. You will assess clients, plan treatments with physicians, document progress, and coordinate care with other disciplines to promote independence and quality of life.

Role emphasizes meaningful one-on-one care, flexible scheduling, and adherence to organizational ethics and agency

Qualifications

• Meets the educational and experience requirements for a Certificate of Clinical Competence in Speech Pathology (ASHA) or is in the process of accumulating supervised experience.
• State licensure/registration as applicable for the practice location.
• Current driver's license and reliable transportation.

Responsibilities

• Assess clients at admission and required time points using appropriate standardized documentation.
• Develop an assessment-based plan of treatment with physician and client; revise as needed.
• Refer for evaluation/care to other disciplines and coordinate care across teams.
