Four Seasons Certified Home Health Agency is seeking a skilled Speech Therapist to provide in-home therapy in Brooklyn, NY. You will work with patients and an interdisciplinary team to enhance communication, speech, and language skills, delivering personalized, evidence-based treatment plans.

You will assess patients in their homes, coordinate with physicians and caregivers, and use modalities such as ultrasound or electrical stimulation.

Qualifications

• New York State Speech Therapist license required.
• Driver's license and a car required.
• Experience in home health or CHHA setting is crucial, with knowledge of OASIS, 60-day episodes, 30-day PDGM and 30-day Reassessment.

Responsibilities

• Coordinate patient care with physicians, therapists, and providers to ensure a cohesive plan.
• Enter visit frequencies and document notes for authorization and Oasis when needed.
• Evaluate patients and develop individualized treatment plans to improve communication, speech, and language.
• Provide hands-on therapy, guide therapeutic exercises, and use modalities like ultrasound or electrical stimulation.
• Train patients and families to use assistive devices and educate on home safety to prevent injuries.
