• Develops treatment plans by identifying problems, goals and methods.
• Documents results of assessments, treatments, follow-up and termination of services.
• Adheres to infection control and professional standards.

Responsibilities

• Optimizes communication, cognitive and swallowing skills, documenting progress toward objectives.
• Establishes individual patient schedules with team members.
• Develops treatment plans with long- and short-term goals and methods.
• Coordinates treatment plan with patient, family and team; implements therapy as outlined.
• Monitors patient response and modifies treatment as needed to achieve goals.
• Recommends and obtains equipment to improve communication; instructs on augmentative devices.
• Instructs patient, family and staff on speech-language procedures or home programs.
• Reviews service quality and efficiency using predetermined criteria.
• Submits billing, payroll and expense documentation as directed and assists in data collection.
• Assists in continuing education and in-service training for staff.
