Job summary

The Roads Home Health Care is seeking a Home Health Speech-Language Pathologist to evaluate and treat patients in their homes across South Florida. You will design individualized treatment plans that help patients regain communication and swallowing function and improve quality of life.

Both full-time and per diem opportunities are available, with collaboration across physicians, patients, families, and the interdisciplinary care team in a compassionate home health setting.

Qualifications

• Requires research-based practice in evaluation and treatment of speech, language, cognitive-communication and swallowing disorders.
• Must be licensed and credentialed to provide home health services in Florida.
• Experience delivering therapy in patients' homes or community settings preferred.

Responsibilities

• Perform in-home comprehensive speech, language, and swallowing assessments.
• Obtain and communicate physician orders for certification of home speech therapy services.
• Deliver individualized speech therapy plans tailored to each patient’s status, needs and age.
