Job summary

CommonSpirit Health at Home is seeking a Speech Language Pathologist (PRN) to assess, diagnose and treat patients in their homes, focusing on speech, language, cognitive-communication and swallowing disorders.

The role involves developing individualized treatment plans, collaborating with families and healthcare professionals, and maintaining compliant documentation to support patient outcomes. A Master's degree, SLP licensure in the state, CCC-SLP and CPR/BLS are required.

Qualifications

• Masters degree in Speech-Language Pathology from an accredited program.
• Active registered SLP license in the state which you practice and certification (CCC-SLP) from ASHA.
• Must obtain and maintain current CPR/BLS Certification
• One-year Speech Therapy experience in a clinical setting.
• Current Drivers License
• Comply with federal, state and local laws and regulations

Responsibilities

• Provide therapy to patients in their homes, including techniques to improve speech, language, cognitive-communication, and swallowing skills.
• Customize therapy strategies to meet individual patient goals and objectives.
• Maintain accurate and timely documentation of patient progress, treatment goals, and outcomes in accordance with regulatory requirements and standards.
• Provide training and support for family members to assist with therapeutic exercise and strategies at home
