Job summary

Community Health at Home is seeking a compassionate Speech-Language Pathologist to join our home-based team in the Indianapolis area. The role involves assessing, diagnosing, and treating patients with speech, language, and swallowing disorders in their own homes, with individualized treatment plans and collaboration with families and healthcare professionals.

The PRN position requires Indiana and Illinois SLP licensure with CCC-SLP from ASHA, CPR/BLS certification, and at least one year of

Qualifications

Master's degree in Speech-Language Pathology from an accredited program.
Active SLP license in Indiana and Illinois and CCC-SLP certification from ASHA.
Current CPR/BLS certification.
One year of speech therapy experience in a clinical setting.
Valid driver's license and ability to travel to patient homes.

Responsibilities

Provide therapy to patients in their homes to improve speech, language, cognitive-communication, and swallowing skills.
Customize therapy strategies to meet individual patient goals.
Maintain accurate, timely documentation of progress and outcomes per regulatory standards.
Train and support family members to assist with exercises and strategies at home.
