Job summary

FOX Rehabilitation is seeking a licensed Speech Language Pathologist to serve older adults in Dayton and/or Beavercreek, OH. You’ll evaluate, treat, and document patient progress in private homes or senior living communities, with the option to incorporate telehealth.

You’ll manage your own schedule, collaborate with an interdisciplinary team, and promote aging in place while delivering high-quality, evidence-based care.

Qualifications

• Valid Speech Language Pathology license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Speech Language Pathology program.
• Basic computer and EMR proficiency.
• Current CPR certification.
• Current Certificate of Clinical Competence (ASHA CCC)

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults wherever they call home (private residences, senior living, memory care).
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, longevity, mobility, and independence.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients and caregivers to promote safety and aging in place.
• Provide telehealth / virtual care when appropriate to extend access and flexibility.
