Job summary

FOX Rehabilitation seeks a licensed Speech Language Pathologist to deliver one-on-one care in Dayton/Beavercreek, OH, including private residences and senior living communities. You’ll evaluate, plan, and treat older adults, with flexibility to integrate telehealth for continuity of care.

Qualifications

• Active Speech Language Pathology license in the state(s) of practice or eligibility to obtain.
• Basic computer and EMR proficiency required.
• Current CPR certification is required.
• CCC credential (ASHA) preferred or required depending on state.

Responsibilities

• Provide one-on-one SLP services to older adults in homes or senior living communities.
• Perform evaluations and develop individualized plans of care.
• Deliver therapy sessions to improve wellness, mobility, and independence.
• Document in FOX EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Offer telehealth/virtual care when appropriate.
