Job summary

FOX Rehabilitation is seeking a licensed Speech Language Pathologist to deliver in-home and senior living care in Beavercreek, OH. You will evaluate patients, create plans of care, and provide evidence-based therapy, often in private residences or community settings.

Clinicians enjoy autonomy with flexible scheduling, telehealth options, and a collaborative interdisciplinary team dedicated to aging in place and quality of life for older adults.

Qualifications

• Valid Speech-Language Pathology license in the practicing state or eligibility to obtain.
• Degree from an accredited Speech-Language Pathology program.
• Current CPR certification and CCC (ASHA).
• Basic computer/EMR proficiency and adaptability to telehealth when needed.

Responsibilities

• Provide one-on-one SLP services to older adults in homes or senior living settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Document in EMR per regulatory standards and collaborate with an interdisciplinary team.
• Educate patients, caregivers, and staff to promote safety and aging in place.
