Job summary

FOX Rehabilitation is seeking a Speech Language Pathologist to provide in-home, senior living, and telehealth services in Port Charlotte, FL. You will evaluate, treat, and monitor older adults in their homes or living communities, building lasting patient relationships while maintaining your own schedule.

Qualifications

• Valid Speech Language Pathology license in the state(s) of practice or eligibility to obtain.
• Degree from an accredited Speech Language Pathology program.
• Current CPR certification.
• Current Certificate of Clinical Competence (ASHA CCC).
• Basic computer and EMR proficiency.

Responsibilities

• Provide one-on-one Speech Language Pathology services to older adults in homes or senior living communities.
• Perform evaluations, develop care plans, and deliver evidence-based treatment.
• Deliver therapy sessions to support wellness, independence, and mobility.
• Document in FOX’s EMR according to regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.
